Gathering Residents to Improve Technology (GRIT) Participant Consent Form

Thank you for your interest in joining a GRIT usability session. GRIT (Gathering Residents to Improve Technology), part of Code for Canada (a federally-incorporated non-profit organization), aims to enhance the usability and accessibility of products, services, applications, and websites through these sessions.

During the session, GRIT staff will facilitate your session, and observe and record information about how you interact with the concepts and prototypes of a project. Members of the research team may also be present to observe your session, benefit from your comments, and thoroughly analyze this session. 

By checking "I Agree," you acknowledge that you understand these terms and agree to participate in the usability session under the terms described below.

Please read these terms carefully before proceeding.  

  1. Participation: Your participation is voluntary and you may withdraw from the session at any time. If you need a break at any time, please inform the facilitator immediately.
     
  2. Data collection: During the session we may collect the following data: 
    1. Audio and video recordings of you in connection with the usability session and the GRIT program. 
    2. Screen captures of your interactions (if applicable).
    3. Responses to questions and feedback you provide during the session. 
       
  3. Use of Data: Code for Canada/GRIT cares about your privacy. The data collected from the usability session(s) will be used solely for the purposes of evaluating and improving the applicable design project. As a default, all of the data and feedback you provide us with in the session will be anonymized by removing any personal identifiers. In other words, your full name or any other information that could identify you will not be used during any presentation of the session results.   4. Compensation: You will receive an incentive for your participation; however, your acceptance of this incentive does not constitute employment by Code for Canada. You may not receive compensation if you do not complete the session.
